Kenyon College Athletics

All varsity athletes are required to have a physical completed prior to competing at Kenyon. Upon arrival you will be required to complete the following forms.

The physical examination will occur on campus with the Kenyon team physician. The date of the exam will be scheduled by the Kenyon Sports Medicine staff.

• Medical Update (PDF) (Returning Athletes)
• Assumption of Risk (PDF) (All Athletes)
• Injury and Illness Reporting (PDF) (All Athletes)
• Medical Release of Information (PDF) (All Athletes)

 
You can complete these forms prior to your scheduled physical.
Blank copies will be available at the physical location.

There is no need to have a physical at home prior to arriving on campus.
